As Project Manager for the Submission Development Stage, ACHIEVE was instrumental in securing a $36M funding agreement involving commonwealth, state and Indigenous health agencies to establish the largest coordinated care project (at that time) in Australia.
This 12 month development stage involved:
- the development of a completely new health and community care delivery framework, including care assessment, care planning, financial modelling, management & IT systems, evaluation and monitoring, and extensive consultation & negotiation.
- extensive consultation with all stake holders including funding agencies, indigenous and mainstream health and community care providers, and the communities involved
- negotiating an agreed framework for the model with all stakeholders
- developing and negotiating the funding framework
- securing the involvement of other consultants and academic institutions
- developming an Action Plan for the implementatiion phase
- Preparing the proposal documentation
The Project thas now been operational for in excess of three years and is currently being built into mainstream service delivery as was the original intention.
